Personal information. When you submit a contact or appointment-request form, we collect the details you provide, such as your name, phone number, and email address.
Health information is collected separately through patient forms and is governed by our HIPAA policies, not this website policy.

We use personal information to respond to your inquiries, provide support, and — where you've opted in — send occasional practice updates. We share it only with vendors that perform services on our behalf, such as email providers.
Non-personal information is used to improve and customize the site and may be aggregated to identify trends.
